Access to Records:

-The privacy and protection of your personal health information is a priority in New Brunswick. When you receive health services, you have a right to expect the appropriate use and safe keeping of your personal health information.

-The Personal Health Information Privacy and Access Act (PHIPAA) provides a set of rules that protects your privacy and the confidentiality of your personal health information. The PHIPAA gives an individual the right to access their own personal health information, that has been collected, used or disclosed by a “custodian” as defined by the Act.

-Your therapist is responsible to notify Service Users and the Information and Privacy Commissioner if identifiable personal health information is stolen, lost, or otherwise inappropriately destroyed, disclosed, or accessed contrary to the Act.

-Service Users entering into service are required to be entered into an electronic information database; this system is confidential and access is limited to authorized staff. Paper Records are kept in a service user’s file in a locked file cabinet. If you wish to view or have access to information contained in your file, please make this request in writing to your therapist and arrangements will be made for you to access the information.

-In order for us to release information about you outside the agency you must first give us permission to do so. This permission must be documented in writing and remains in your client file. At any time, you may revoke the consent to release information (this must also be documented in writing).
